Claiming a TOG care allowance

You have a child aged 3 or older and under 18, and he or she has a severe illness or a disability. You can submit a claim for a TOG allowance via the internet. You will then be guided through the form step by step on your computer screen. You can sign and send off your claim using your DigiD code. (You must also send us a copy of your child's AWBZ indication by post).

You can also download a TOG claim form from this page and print it out. You can then send us the completed form by post. Alternatively, you can request the form from our TOG department in Roermond. They will send it to you straight away. Their telephone number is (0475) 36 80 40.

Please send the completed form, together with a copy of the AWBZ indication, to our office in Roermond, using the address: Postbus 1244, 6040 KE Roermond.

When will your TOG allowance start?

The TOG allowance always starts on the first day of a quarter: on 1 January, 1 April, 1 July or 1 October. If you meet the conditions on one of these dates, you will receive your TOG allowance as from that date. If you do not meet the conditions until after these dates, your TOG allowance will be paid as from the next quarter.
The situation on the first day of the quarter is not relevant for an AWBZ indication. The indication has to be valid, and the SVB must receive it, in the quarter concerned. You can then receive a TOG allowance as from that quarter (provided you have also submitted a claim).

Example
You send in your claim on 25 June because your child has turned 3. Your child has a severe illness and lives at home with you. The AWBZ indication is valid as from 2 September. You can get a TOG allowance as from the third quarter (as from 1 July).
