You can claim a TOG care allowance in different ways. Choose how you wish to submit your claim.
Submitting a claim with DigiD
You can fill in the claim form on your computer screen. You will be guided through the form step by step. Using your DigiD code, you can sign and submit your claim online.
Step by step on your computer
You can fill in the claim form on your computer. We will guide you through the form step by step. When you have completed the form, you can print it out, sign it and send it to us. You must also complete and send the medical form and the ClientFirst form.
Download and send the form
You can download the claim form, fill it in, and send it to us by post. The form has been saved as a PDF file. To read PDF files, you need the program Acrobat Reader. You must also complete and send the medical form and the ClientFirst form.
