Co-insurance and income from the Netherlands

If you live in Belgium and work in the Netherlands

If your partner and/or children do not have income of their own, they can be covered under health insurance in the Netherlands. For this, you have to register them with your health insurance fund in Belgium using form E106, which you can obtain from your Dutch health insurer. You then submit the form to your health insurance fund. They will establish which family members qualify for co-insurance with you and they will inform the Dutch Health Care Insurance Board (CVZ) (CVZ).

Your co-insured family members can receive medical care in Belgium. The health insurance fund will reimburse the costs of the care in accordance with the rates applicable in Belgium. The health insurance fund will then be reimbursed by the Netherlands. 

Co-insured family members can also receive medical care in the Netherlands. They do not need permission from the Belgian health insurance fund for this. Agis Zorgverzekeringen will reimburse the costs of the care. As from 1 January 2012, your co-insured family members can use the European Health Insurance Card - EHIC) to receive care in the Netherlands. With this card, the care provider can settle the costs of treatment directly with Agis. Each family member must apply to the CVZ for his or her own card. Family members aged 18 and over must pay a fixed Zvw contribution for non-residents of € 71.57 per month to the CVZ. Your family members may be able to get a healthcare allowance from the Tax Administration, Belastingdienst/Toeslagen. Children aged 18 and over can apply for the healthcare allowance themselves.

If you live in Belgium and receive a pension or benefit from the Netherlands

If you have a partner and/or children without income of their own, they can also be covered under health insurance in the Netherlands. For this, you must register them with your health insurance fund in Belgium using form E121, which you can obtain from the Dutch Health Care Insurance Board (CVZ). You then submit the form to your health insurance fund. They will decide which family members can be co-insured with you, and they will inform the CVZ.

Your co-insured family members can receive medical care in Belgium. The health insurance fund will reimburse the costs of the care in accordance with the rates applicable in Belgium. The health insurance fund will then be reimbursed by the Netherlands.
