Co-insurance and income from Belgium
You live in the Netherlands and work in Belgium
If you have a partner and/or children up to age 17 without income of their own, they can also be covered under health insurance in Belgium. For this, you must register them with the health insurer CZ Zorgverzekeringen using form E106 or S1, which you can obtain from your health insurance fund in Belgium. You then submit the form to CZ. They will decide which family members can be co-insured with you, and they will inform your health insurance fund.
Your co-insured family members can receive medical care in the Netherlands. CZ will reimburse the costs of the care in accordance with the rates applicable in the Netherlands. CZ will then be reimbursed by Belgium. Your family members will not have to pay a fixed Zvw health insurance contribution to CZ. This means that they cannot get a health care allowance from the Dutch Tax Administration (Belastingdienst/Toeslagen). For more information about the rights of co-insured family members, see the 'buitenlandtool' on the website of CZ (available in Dutch).
Co-insured family members can also receive medical care in Belgium. For this, they do not need permission from CZ. Your health insurance fund will reimburse the expenses. Your family members will pay a contribution in Belgium called 'lidgeld' to your health insurance fund. The amount of this contribution varies between the different health insurance funds. It will be between € 5 and € 8 per month.
