Medical insurance for your family
If your family members have income of their own, they will be insured in the country from which they receive that income. If they do not have income of their own, please read the information below.
Family members without their own income
If your family members do not have income of their own, they can be co-insured with you for medical care.
If you work in the Netherlands
If your partner and/or children do not have their own income, they can be covered under health insurance in the Netherlands. For this, you have to register them with your health insurance fund in Belgium using form E106, which you can obtain from your Dutch health insurer. You then submit the E106 form to your health insurance fund. They will then establish which family members qualify for co-insurance with you and they will inform the Dutch Health Care Insurance Board (CVZ).
If you receive a pension or benefit from the Netherlands
If you have a partner and/or children without income of their own, they can also be covered under health insurance in the Netherlands. For this, you must register them with your health insurance fund in Belgium using form E121, which you can obtain from the Dutch Health Care Insurance Board (CVZ). You then submit the form to your health insurance fund. They will decide which family members can be co-insured with you, and they will inform the CVZ.
If you work in Belgium
If you have a partner and/or children without income of their own, they can also be covered under health insurance in Belgium. For this, you must register them with the health insurer CZ Zorgverzekeringen using form E106 or S1, which you can obtain from your health insurance fund. You then submit the form to CZ. They will then decide with family members can be co-insured with you, and they will inform your health insurance fund.
If you receive a Belgian pension or benefit
If you have a partner and/or children without income of their own, they can also be covered under health insurance in Belgium. For this, you register your family members with the health insurer CZ Zorgverzekeringen using form E121 or S1, which you can obtain from the institution that pays your pension or benefit. You then submit the form to CZ. They will then decide which family members can be co-insured with you, and they will inform your health insurance fund.
